Project Manager LGBTQ Program

 

Position:  Project Manager LGBTQ Program

(Contract till March 31, 2019) (.2 FTE, 1 day a week)

Reports To:  Clinical Program Manager

Position Summary:

As a key member of the Couchiching Family Health Team (CFHT), and an integral part of an interdisciplinary health care team, the major role of the Project Manager for the LGBTQ Program will be to coordinate clinical team development, training, education, program coordination, organize team building and meetings, site selection, program agreements between all parties, assist in development of evaluative aspects to the program including the Quality Improvement Plan.

Roles and Responsibilities:

  • Oversee the execution, implementation, evaluation of all stages of the project
  • Adhere to the highest QI standards
  • Oversee the development of program policy and procedure documents
  • Manage all statistical reporting and data gathering to ensure project is meeting projected targets and act as the key point of contact for implementation
  • Maintain knowledge of local and provincial Health Service Providers (HSPs)
  • Lead discussions regarding project implementation among project team and external HSPs
  • Develop a detailed project plan to reach all key deliverables by fiscal year-end
  • Communicate status of project to various working groups and key partners, as well as boards of local HSPs
  • Arrange on-going communication events for key partners
  • Work with and engage community partners to support implementation
  • Participate in information sessions and knowledge transfer sessions
  • Develop a plan for on-going sustainability post implementation
  • Identify issues and risks and report these to CPM
  • Report on project milestones and timelines and participate in regular status update meetings
  • Delegate to team members appropriate project co-ordination
  • Coordinate and lead discussions with local HSP’s to better understand their needs and future plans
  • Supervise to ensure project deficiencies are identified and addressed in a timely manner
  • Continually update project software to ensure on time with tasks
  • Participate in weekly touch base meetings with core team, as well as regular working group meetings, community of practice meetings, and steering committee meetings
  • Implements efficiency through the use of technology.
  • Travel between HSPs as required.
  • Maintain current knowledge of policies and procedures
  • Maintain confidentiality in accordance with the Municipal Freedom of Information and Protection of Privacy Act
  • Work in compliance with Health and Safety laws adhering to safe work practices and procedures established by the CFHT
  • Other duties as required

Education/Specific Job Requirements:

  • University degree in Health Sciences
  • Able to work flexible hours including evenings if required
  • Police Record/Vulnerable Record Check

Experience:

  • 3-5 years project management experience in a health care environment
  • Experience with an Electronic Medical Record (EMR)
  • 5+ years experience implementing projects in a health care environment
  • Demonstrated experience in facilitation, collaboration and negotiation with multiple stakeholders and service providers
  • Experience managing and coordinating project teams, contract and external resources
