Quality and Improvement

On an annual basis, Couchiching Family Health Team is required to submit a Quality Improvement Plan (QIP), to Health Quality Ontario.

A Quality Improvement Plan is a public commitment to meet quality improvement goals. By developing the plan, the organization outlines how they will address improving the quality of care it provides to its patients, residents or clients.

A Quality Improvement Plan is a documented set of quality commitments made by a health care organization to its patients, clients, residents, staff and community on an annual basis. The goal is to improve quality through focused targets and actions.
