Feedback regarding Customer Service Accessibility for Ontarians with Disabilities or any questions about our policy or its related procedures should be directed to Francine Kerr, Human Resources Manager, using the contact information below.

Documents related to the Accessibility Standard for Customer Service are available upon request and in a format that takes into account the person's disability and are available on our website at www.cfht.ca. Our Accessible Customer Service Policy and its related procedures will be reviewed as required in the event of legislative changes.

The feedback will be presented to the Chief Executive Officer and the Patient/Client will be contacted to inform them of the result and changes if any. This process will be recorded. 

It is our expectation and requirement that all participants in Couchiching Family Health Team Programs (clinical staff, agencies, patients and family members) conduct themselves in a courteous, respectful and cooperative manner both in person and on the telephone. Any person unable to meet this expectation will not participate in further Couchiching Family Health Team interactions. Our goal is always to ensure our patients receive the best care possible and this can only be achieved if we collaborate respectfully together.
